چکیده
Cardiovascular implantable electronic devices (CIEDs) are common in the United States as treatment for cardiac rhythm problems. These devices can be lifesustaining. As patients age and suffer from worsening cardiac-related disorders and other terminal illnesses, the electrophysiology community can expect increased requests for device deactivation from patients and their surrogates, creating tension between competing ethical obligations to respect patient autonomy, on the one hand, and to promote patient well being (“beneficence”) and avoid harm (“nonmaleficence”), on the other.

Should implantable cardioverter-defibrillators and permanent pacemakers in patients with terminal illness be deactivated? Deactivating Implantable Cardioverter-Defibrillators and Permanent Pacemakers in Patients With Terminal Illness
The recently published 2008 Guidelines for Device-Based Therapy of Cardiac Rhythm Abnormalities1 include a discussion of ethical issues surrounding the deactivation of pacemakers, implanted cardioverter-defibrillator (ICDs), and CRT devices in patients who are terminally ill. The authors rightly note that most clinicians make an ethical distinction between deactivating an ICD and deactivating a...

متن کاملErosive Pressure Dermatitis Associated with Cardiac Pacemaker
Erosive dermatitis is a skin reaction associated with implantable cardiac devices (ie, pacemakers and cardioverter defibrillators). We report a 65-year-old man with a history of several cerebrovascular attacks who developed an erythematous crusted erosive patch over the pacemaker site after implantation of two subsequent pacemakers. We suggest that physical irritation may be a trigger factor fo...
متن کاملStaphylococcus aureus bacteremia in patients with permanent pacemakers or implantable cardioverter-defibrillators.
BACKGROUND Although cardiac device infections (CDIs) are a devastating complication of permanent pacemakers or implantable cardioverter-defibrillators, the incidence of CDI in patients with bacteremia is not well defined. The objective of this study was to determine the incidence of CDI among patients with permanent pacemakers or implantable cardioverter-defibrillators who develop Staphylococcu...
متن کاملTHE ROLE OF PACEMAKER CLINIC IN THE FOLLOW-UP OF PATIENTS WITH PERMANENT PACEMAKERS
The Pacemaker Clinic at Shahid Rajai Cardiovascular Research Center first started operating in August 1992. In its first year of operation, 294 permanent pacemaker patients were studied, allowing an appraisal to be carried out of the clinic's effectiveness in diagnosing and treating early complications. In the final analysis, pacemaker complications were observed in 68 (23%) patients, of wh...
